Our physical health and social situation can affect our wellbeing. Sometimes we all need some practical or emotional support to help us to cope.
The Positive Pathways Service supports adults in the community on a short-term basis to stay well and thrive. We accept professional referrals from GP surgeries, Mental Health Practitioners, Social Prescribers and NHS Community Mental Health Teams.
Positive Pathways starts with a conversation about “what matters to you” including what’s good and what could be better. Together we create a wellbeing plan and identify the steps to take. Our team can share links with local organisations and opportunities who can help such as benefit advice, housing, social activities, debt management and counselling. We can accompany people when they start something new if they are anxious.
We work across South West Essex. We have a team based in Thurrock that support people in Thurrock. Our Brentwood based team also support people in Basildon, Billericay, Wickford and surrounding areas. Available Monday to Friday 9am-5pm with evening and weekend appointments available when needed.
